The Hospital Under the Island

The hallway outside the elevator smelled like death.

Not old death.

Fresh death mixed with seawater and rust, thick enough that Nora Vale physically struggled not to gag the second she stepped into the corridor.

Water dripped steadily from exposed ceiling pipes above them while dim red emergency lights flickered weakly across the underground floor. Every few seconds the lights buzzed loudly like they might die completely at any moment.

God.

This place should not exist beneath a resort.

Kai Mercer stared at the rusted hospital beds lining the walls.

“I would just like to officially say,” he whispered carefully, “I hate every single decision we’ve made tonight.”

Nobody disagreed.

The writing across the far wall still dripped slowly downward like it had been painted recently.

THE OCEAN TOOK THEM FIRST

Selene wrapped her arms tightly around herself while staring at it.

“What does that even mean?”

Before anyone answered—

a loud metallic clang echoed somewhere deeper underground.

Everyone froze immediately.

The sound came from farther down the corridor beyond the darkness.

Like something heavy falling over.

Then silence returned.

Jace lifted the flashlight carefully.

“We move quietly.”

Kai looked emotionally offended.

“We should move BACK to the elevator.”

The elevator behind them suddenly dinged softly.

Every head turned instantly.

The doors slowly opened again by themselves.

The cabin stood empty.

But muddy wet footprints led out from inside the elevator and disappeared down the opposite hallway.

Fresh footprints.

Something came down after them.

God.

Nora’s pulse immediately quickened.

“We’re not alone down here.”

“No kidding,” Kai whispered.

Then suddenly—

the elevator lights died.

The doors slammed shut hard enough to echo through the basement.

And the elevator began rising upward again without anyone inside.

Leaving them trapped underground.

Selene stared after it in horror.

“Oh my God.”

Another clang echoed deeper inside the basement.

Then came a sound that froze every person in the hallway instantly.

Coughing.

Weak human coughing somewhere in the darkness ahead.

Alive.

Nora looked toward the others immediately.

“You heard that, right?”

Jace already started moving forward.

Of course he did.

Kai grabbed his arm briefly.

“If we die because you’re heroic, I’m haunting you.”

“Fair.”

The group moved deeper through the corridor carefully while water splashed softly beneath their footsteps. Every room they passed looked worse than the last.

Old patient rooms lined the hallways.

Most doors hung open revealing rusted medical equipment and mold-covered walls inside.

But some rooms looked occupied recently.

Candles burned low inside several corners.

Blankets rested on beds.

And written repeatedly across one wall in frantic handwriting were the words:

DON’T FALL ASLEEP AFTER MIDNIGHT

God.

Nora’s chest tightened.

Because whatever happened here…

people survived long enough to leave warnings behind.

Another cough echoed nearby.

Closer now.

The group followed the sound carefully until they reached a pair of large steel doors near the end of the corridor.

One door stood partially open.

Dim lantern light flickered from inside the room beyond.

And someone was breathing heavily inside.

Jace slowly pushed the door wider.

The room looked like an old underground medical ward converted into a survival shelter.

Mattresses covered the floor.

Lanterns glowed weakly beside stacked canned food and water bottles.

And sitting against the far wall beneath hanging medical curtains—

was a man holding a shotgun.

He looked around forty years old with a thick beard and bloodshot eyes hollowed by exhaustion. A deep scar ran across one side of his neck disappearing beneath his collar.

The second he saw the group enter—

he raised the shotgun instantly.

“DON’T MOVE.”

Everyone froze.

Kai slowly lifted both hands.

“Okay wow, finally a normal terrifying person.”

The man’s eyes darted wildly across the group.

“You came from the ferry?”

Jace nodded carefully.

“Yes.”

The man went pale.

God.

Actual fear crossed his face.

“How many survived?”

Nobody answered immediately.

Because honestly?

They didn’t know anymore.

Finally Nora whispered:

“Just us.”

Silence filled the underground room.

The man slowly lowered the shotgun afterward while exhaustion settled visibly across his face.

Then softly, almost to himself:

“They’re taking more every storm now.”

Selene frowned immediately.

“What are you talking about?”

The stranger looked toward them carefully.

And for the first time, Nora noticed how terrified he truly looked.

Not normal fear.

The kind of fear people carried after surviving something impossible too many times.

The man stood slowly.

“My name’s Elias.” His voice sounded rough from exhaustion. “And if you want to stay alive until morning…” He looked toward the dark hallway outside. “You need to understand this island before midnight comes.”

Kai swallowed hard.

“What happens at midnight?”

Elias didn’t answer immediately.

Instead, he walked toward one of the old hospital windows covered by metal bars.

Beyond the glass lay only darkness and crashing ocean far below the cliffs.

Then quietly—

almost like he was ashamed of the words—

he whispered:

“That’s when the drowned start feeding.”

Silence swallowed the room completely.

Thunder rumbled faintly somewhere above the island.

And deep below the underground hospital—

something massive moved beneath the earth.
